The custom tattoo intake sheet template

A one page sheet you fill in for every custom inquiry, so nothing about size, placement, budget or terms is still vague when you start drawing.

Most bad tattoo bookings are not bad clients. They are bookings where size was agreed in adjectives, placement was never photographed, and the budget conversation happened after the drawing was finished. A one page intake sheet fixes that by asking the same questions in the same order every single time.

This template covers the four stages of a custom commission: what you need before writing a real reply, what you confirm on the consultation, what gets locked before the stencil, and what leaves with the client. Fill one in per client and keep it filed with the consent form.

How to use it

Print the sheet and keep a stack by the station, or keep it open on a tablet while you talk. The point is that every custom inquiry leaves the same trail: references, placement photo, size in inches, budget range, medical flags, agreed sessions, deposit taken, terms accepted, aftercare given.

When a client disputes a charge, when an inspector asks for a record, or when a piece turns into three sessions instead of one, the filled sheet is what you reach for. Artists who run it consistently also find their quoting gets faster, because the same six answers set the price nearly every time.

Send me the editable copy

Send yourself the editable copy by email and rewrite the deposit and aftercare lines to match your own studio.

There is no newsletter behind this and no list you are joining. You get the editable intake sheet and nothing after it, and if you would rather skip the form and just ask how the deposit or aftercare lines should read for your studio, write to jimenezjulien42@gmail.com and a person will answer.

Questions about this document

Do I need StencilIntake to use this sheet?

No. The template works on paper and it works on a tablet. StencilIntake runs the same sequence for you and stops the deposit and aftercare steps from being forgotten, but the sheet stands on its own.

Does this replace my consent form?

It does not. Consent, medical history and the records your state requires stay separate documents you keep on file. Treat this sheet as the scoping and terms record that sits alongside them.

Can I put my own studio wording on it?

Yes, that is the intention. Deposit amounts, notice windows, session lengths and aftercare wording differ by studio and by state. Ask for the editable copy and rewrite those lines in your own voice.

Running this sheet on every tattoo commission

Filling in references, placement, sizing, medical flags and deposit terms for one client is easy on a quiet Tuesday. Doing it identically for every inquiry through a week of long sessions, touch ups and a guest spot is where studios slip, and the step that gets skipped is almost always the written terms or the aftercare copy. StencilIntake runs that sequence for you and sends the aftercare wording when you close out the session. Book a demo and we will walk it through on a real piece.
